1. Astellas Pharma US, Inc. is sponsoring a scholarship contest in which winning transplant recipient and organ donor entrants will be awarded a $5,000 scholarship for use toward education expenses. Winners will be ineligible in any future years' contests.
2. No purchase necessary. Contest open only to liver, kidney, or heart transplant recipients taking Prograf^® (tacrolimus capsules) or to organ donors who have donated a portion of their liver or a kidney. Entrants must be legal US residents and 13 years of age or older.
3. Participants must submit an entry form and a brief essay in English of no more than 500 words, describing how their transplant or donor experience has inspired their educational plans. Entries can be submitted electronically at www.AstellasTransplant.com, but must not be received later than 11:59 pm ET on June 25, 2010. Entries can also be sent by mail, typed, to Transplant Scholars Award, 534 4th Street, San Francisco, CA 94107, postmarked no later than June 25, 2010. Additional entry forms may be obtained by visiting www.AstellasTransplant.com. All contest winners will be notified via phone and mail; the winners will also be announced on www.AstellasTransplant.com on September 15, 2010.
4. Essays will be judged by a panel based on: (a) the compelling nature of the story; (b) the educational goals of the applicant and how those were affected by transplantation; and (c) the applicant's intention to positively impact the transplant community. The decision of the judges is final.
5. Employees of Astellas Pharma US, Inc., its agencies, affiliates, and subsidiaries, and their respective family and household members, are not eligible for participation.
6. Winners must sign and return a Release of Liability and a Certificate of Original Work and consent to the use of their names and/or likeness in advertising and participation in media interviews (where not prohibited by law) within 14 days of notification or prize will be forfeited and offered to an alternate winner.
7. All submitted essays and photos become the property of Astellas Pharma US, Inc.
8. The winners agree that Astellas Pharma US, Inc. and its affiliates and their officers, directors, employees and agents, successors, and assigns shall have no liability whatsoever for any injuries, losses, or damages of any kind resulting from acceptance, possession, misuse, or use of the prizes.
